Patient-Centered Needs Assessment

Needs assessment is an essential foundation of CHESS. It drives everything we do, from determining what services to provide within a CHESS module, to what content should be included, and how we implement CHESS.

There are four components to the needs assessments we conduct:

The first three activities help us identify patients' and their family's needs. Surveys allow us to measure the importance of those needs and how well those needs have been satisfied by existing services.
