NIATx Wisconsin Mental Health Readmissions Collaborative
Overview
The NIATx Wisconsin Mental Health Readmissions Collaborative provides a tailored framework for reducing inpatient readmissions in designated counties in Wisconsin.
In each 12-month collaborative, participating counties use the NIATx model to examine and improve the hospital discharge process.
The project began in 2010 and as of 2014 had reduced 30-day readmissions by 20 percent, increasing access to service for more people and reducing costs for the state's publicly-funded mental health system.
