Wisconsin community health centers and community-based behavioral health organizations are invited to participate in a five-month technical assistance project (February-July 2012) to increase collaboration between primary care and behavioral health providers in order to deliver holistic, integrated care for individuals.
The focus of this effort is on collaborations between federally qualified health centers and behavioral health agencies offering substance abuse services. To apply, it is not necessary to have an established relationship with a behavioral health agency or a federally qualified health center.
The goal of this project is for participants to create and/or strengthen a mutually beneficial working relationship between primary care and behavioral health entities to improve the quality of care they deliver to their patients/clients.
Please complete the application form and submit via email to carol.sherbeck@chess.wisc.edu
