Sustainability Model
The Sustainability Model (SM) is a collaboration between the British National Health Service and the University of Wisconsin.
Using proven models of change and decision analysis, the SM was developed by a panel of nationally respected practical and theoretical experts in change and has been tested for accuracy in over 200 separate change projects.

Definitions:
- Champion: Person assigned to make this project succeed
- Senior leader: People who give approval to do the projec
- Target group: People who will have to change their behavior
- Customers: People who receive the product or service
