Incentives for Spreading Process Improvement (ISPI)

In an ISPI system, states reward providers for spreading process improvement efforts in addition to offering incentives for achieving process improvement targets. Oklahoma’s ISPI program, designed as a part of its STAR-SI project, rewards providers for achieving specific set targets related to:

Providers have the opportunity to complete their own STAR-SI Incentive Activity Proposal based on these three incentive categories. They are rewarded for meeting the target, not punished for a failure to achieve their goals. Read more about the Oklahoma incentive program Add to portal.

Following the NIATx principle of rapid-cycle testing, pilot testing the system with a core set of providers allows a state or other payer to rehearse the system before introducing it state or system wide.

Consider pilot testing by:

  • Involving only a core set of providers
  • Allowing providers opportunities to review contract changes
  • Eliminating accountability for improvement changes (good or bad) for the first six months
