All selected participants are required to attend.
May 2012: Closeout session, AATOD national conference (dates TBA)
How to Apply
Organizations that wish to participate in the NIATx Opioid Treatment Provider Process Improvement Learning Collaborative, Cohort II must:
- Conduct a walk-through. Click here for instructions.
- Identify key staff who will participate in the project. Click here for instructions.
- Complete the application form.
- Submit applications by close of business on April 29, 2011, via e-mail to: jeanne.pulvermacher@chess.wisc.edu
